Medical Records How to get your medical records This page provides the information for requesting your Medical Records from your doctor(s) with St. Elizabeth Physicians. Option 1: Evaluate if the medical record information available to you in MyChart meets your needs. You can download your MyChart records for personal use and/or send them to a provider. You can access your MyChart records anytime free of charge. MyChart provides patients with current health Issues, medications on file, allergies, immunizations, completed test results, and medical history on file electronically (medical history, surgical history, family medical history, and social history). If you have a MyChart account,click the image above to login. Don't have a MyChart account? Sign up now. Option 2: Submit a request through MyChart. Records are released through MyChart for free. Records released through MyChart are only the records that are stored electronically. Select the "Request your Doctor's Office Records" option to request your medical records from your doctors with St. Elizabeth Physicians. Option 3: Complete the AUTHORIZATION FOR USE OR DISCLOSURE OF PROTECTED INFORMATION form. Submit the signed authorization form to St. Elizabeth Physicians Central Medical Records department using one of the three options below. Please only choose one; submitting through multiple channels slows down the process. Click here if you would like a printable version. You can also obtain this form by visiting your local St. Elizabeth Physicians office. How long will it take to get my records released? Records release can take between 20 to 30 days to complete. St. Elizabeth Physicians started using electronic medical records in 2010. Paper records before that time are stored off-site; please allow additional time if requesting records from your paper chart. Requesting records through MyChart will take less time because it is all electronic. Where are my Medical Records? You can contact our Medical Records department at (859) 905-3910 to inquire about your paperwork. Please wait 10 business days after submitting your release of information form to us to allow time to process your paperwork. Can you fax my records to me? No, the Central Medical Records department does not fax records. Is there a fee associated with requesting my medical record? Requesting records through MyChart is free. St. Elizabeth Physicians uses CIOX Health to release medical records not requested through MyChart. Per CIOX Health, "These prices include patients’ directives to send their records to a third party. It will not include requests from attorneys, insurers, or others which require a patient authorization. CIOX Health is adopting, effective August 23, 2016, a patient flat rate of $6.50 for records delivered electronically that are stored in an electronic health record. For records that are stored on paper that must be scanned in order to make them electronic or in order to reproduce them for paper delivery, the patient rate will be $6.50 plus $.02 per page for the cost of scanning. If a record is stored on paper and delivered on paper, the price will be $6.50 plus $.02 per page for the cost of scanning plus $.05 per page for the cost of paper and toner. Actual postage will be billed for any mailed records." By law, Kentucky residents will receive their first copy free of charge. Indiana and Ohio residents will be subjected to the above fee structure for all requests. Additional costs may vary. I received a bill from a company outside of St. Elizabeth Physicians. Is it legitimate? St. Elizabeth Physicians uses a company by the name of CIOX Health to process and complete medical record releases. Will my requested record include all information from St. Elizabeth Physicians and St. Elizabeth Healthcare? No. You will only receive records generated by St. Elizabeth Physicians. St. Elizabeth Physicians cannot release the medical records from St. Elizabeth Healthcare. If you would like records from the hospital side of St. Elizabeth, you will need to contact their Medical Records Department at (859) 301-2240 or submit a request through MyChart by selecting the "Request your Hospital Record" option. I am the Executor, Executrix, or the Administrator of the Estate of my deceased loved one; do I need legal documentation attached to the signed authorization form? Yes. Please attach a copy of the legal documentation, not the original, Executor/Executrix document signed by the judge, Administrator document signed by the judge.
